Application for PCS Fellowship
The College accepts application for membership until April 30, 2010
Requirements are as follows:
Accomplished Application Form & Pictures
Medical School Diploma (Certified true copy)
Medical Board Certificate (Certified True copy)
Certificates (Residency/Fellowship) (Certified true copy)
Certificate of membership in Good Standing (PMA)
Specialty Board Certificate (Certified true copy)
Specialty endorsement
Letter of Recommendation (2 Fellows)
Letter of Hospital Director/Chairman/Chief of Specialty
(3) 2x2 picture (white background)
Please submit completed forms at the PCS Secretariat, Committee on Membership, c/o Eden Grace A. Paule, 3rd Floor PCS Bldg., 992 EDSA Quezon City on or before April 30, 2009
